Scott Forman, MD
Founder and Chief Executive Officer
ThirdRockMD was founded by Scott Forman, MD, MSc, FAAEM in 2016. A board-certified emergency medicine physician, Scott is a former international high-altitude mountain guide, with an extensive background in austere and expedition medicine, marine and tropical medicine, flight medicine, and both wilderness and urban search and rescue. He continues to practice in all these areas.
Scott has a BA in Philosophy and English from Northwestern University, earned his MD and an MSc in epidemiology and diving pathophysiology from St. George’s University, and completed his emergency medicine residency training at University of New Mexico. Scott has practiced emergency medicine in a range of clinical environments from level 1 trauma and tertiary care centers to sole-provider, rural critical access centers. He has also served patients on Navy hospital ships, as a flight physician, and in remote clinics on all seven continents.
When not working or training and competing in endurance events, Scott enjoys traveling the world and epic adventuring with his friends, wife, and son.

Jay Debonis
Chief Operations Officer; Chief Strategy Officer
Jay has more than 25 years experience in operations, innovative analytics, and product development across multiple industries. The last 11 years has been in the healthcare field leading healthcare operations; launched innovative business solutions in operational, clinical, and enterprise sustainability; and designed predictive/scenario modeling with prescriptive analytics to transform healthcare in anticipation of future organizational need.
Jay has a BS in Mechanical Engineering from the University of New Mexico.
Jay enjoys reading, traveling and sharing a burger and beverage with friends from around the world.

Ian (Jamie) Jamieson
Director of Operations
Ian (Jamie) Jamieson is a highly accomplished leader with over two decades of experience in military special operations and civilian medical consulting. He excels in strategic planning, operational management, and risk mitigation and is highly adaptable in dynamic environments.
Jamie retired after more than 21 years as an SMSgt in the USAF. He served as a pararescueman in special operations; his last assignment was Mission Support Superintendent for the Oregon Air National Guard, where he oversaw 60 personnel and seven work centers, ensuring the force could deploy worldwide at a moment's notice.

Ian Butler-Hall, MD
Chief Medical Officer
Ian is a board-certified emergency physician with a subspecialty board certification in EMS. He graduated from Duke University with a degree in
religion, then attended medical school at the University of Washington.He completed his emergency medicine residency at Loma Linda University Medical Center in Southern California where he also did a fellowship in EMS. Ian has been a medical director of EMS agencies for nearly 15 years; he has also served on rescue helicopters and been a race physician for the Baja 1000.
Ian and his family recently spent a year and a half living and working in New Zealand. Ian enjoys international travel, skiing, and running marathons. He lives in Boise, Idaho, with his wife (also an emergency physician) and two teenage children.

JAMES LIND, MD
Medical Director, Pan-Pacific Region
James has been an emergency medicine physician in Queensland, Australia for over 22 years and works as a clinical lead in the Australian Department of Health helping to solve critical issues across Queensland. James has experience in the prehospital arena, serving in helicopter rescue and deployment for disaster medicine. He has also served as a concierge medicine physician and provided medical support for sports teams, entertainers and other high net worth individuals and their families.
James has a BMBS in BioMedical Sciences from the University of Nottingham and FACEM in Emergency Medicine from Gold Coast Hospital. An avid basketball fan, James still finds time to play in a local league.

Josiah Blanton
Training Program Lead
Josiah brings unparalleled experience, dedication, and leadership to his role as Training Program Lead. He served honorably for 20 years in the United States Air Force as a Pararescueman, conducting numerous rescue operations, providing life-saving medical care to military personnel in combat zones, and supporting domestic rescue missions.
Following his military service, Josiah transitioned to law enforcement, serving as a Deputy Sheriff in Washington State. While on the SWAT team, he acted as the primary Tactical Emergency Medical Support (TEMS) medic. His work since has included working as a close protection agent and field medic, traveling globally to support various organizations.
Josiah is a nationally registered Emergency Medical Technician–Paramedic and holds multiple state paramedic licenses.

CATHERINE FORMAN
Communications and Strategy; Leadership Advisory
Catherine has spent over 30 years working in corporations and as a consultant across a variety of internal and external communication responsibilities. She brings an analytical approach, business acumen and strategic thinking to communication, operational and marketing challenges, developing and delivering strategic solutions that drive business results.
Catherine has a BA in Economics from the College of William and Mary, and MA in Economics from Stanford University, and is currently working toward a Master’s in Public Health at the University of New Mexico.
At one point an avid rock climber and high-altitude mountaineer, Catherine’s outside activities are somewhat tamer these days, but she continues to run, hike, dive, and paddleboard, and is always updating her list of destinations for her next adventure.

KIM WOOLLEY
Mental Health Services
Kim is a Licensed Clinical Social Worker (LCSW) who developed her career over two decades at nationally prominent hospitals including Stanford and UCSF. She has significant experience helping individuals reduce stress and address issues including bereavement, substance abuse, PTSD and depression, leading to improved team cohesion, greater consistency, and higher quality team performance.
Kim has a Bachelor’s degree and a Master’s degree in Social Work from Arizona State University.
Kim is a stand-up paddle board aficionado, is passionate about horseback riding, and devotes significant time and energy to Rock Steady Boxing SF, a program for people with Parkinson’s disease.

JOHN HULLVERSON
General Counsel; Leadership Advisory
John is an attorney with more than 25 years of experience in the medico-legal field with an emphasis on risk assessment and risk mitigation aimed at improving patient outcomes and quality of care.
John’s legal counsel is augmented by his interest and experience in prehospital emergency medical care. He has been a licensed EMT since 1996, interned with a helicopter medevac service, and holds additional certifications including Wilderness EMT and Diver Medic, board-certified by the National Board of Diving and Hyperbaric Medical Technology.
John is a 2-time US national record holder in the sport of freediving. He also trains big wave surfers and other watermen/women in breathold surf survival and serves as a deep safety freediver in international competitions.
